Application Form
Please note that it is required for out-of-town teams to stay at authorized tournament hotels to participate.
This form is for application to participate in the Midwest Hockey Tournament Series. Acceptance is contingent upon approval of the
Tournament Director. Upon receipt of your application, Midwest Hockey Tournaments will hold a spot for your team for 5 days. If the
registration payment is not received, your team will be placed on the waiting list. Remit entry fee as posted on individual tournament
webpage. (Mail entry fee to tournament contact as listed.) Entry fee must be paid in full along with tournament application form and is non-
refundable once the Tournament Director accepts the team into the tournament.

All applications must be received by application deadline as posted on individual tournament webpage. Selection of teams will be made
after application deadline. The following information is requested for team selection:

  •        Completed tournament application
  •        Team Tournament Roster
  •        Tournament Fee (Check made payable to the Hoffman Estates Park District)

If your application is not accepted, a full refund will be provided. Once accepted, out-of-town teams must provide their Hotel Rooming List
to the tournament director three weeks prior to the tournament date.

All teams must be registered with USA Hockey. Teams will be required to submit a valid USA Hockey roster.
